CWP ORIENTATION
This electronic registration/enrollment form is for new CWP members. The following video introduces how to fill out the enrollment form. Please only complete this form if you have been requested to do so by a CWP worker. The CWP navigator assigned to assist you will be able to access your responses after you click "submit".
Please make sure you connect over the phone with your Engagement & Resources Navigator on the date of your scheduled Orientation / Intake. Talking to them is critical to moving forward with CWP.
