
A Valued Partnership
The Community Works Project (CWP) partners closely with the Oregon Department of Human Services (ODHS) to support Temporary Assistance for Needy Families (TANF) recipients on their journey toward self-sufficiency. This partnership is designed to provide holistic, individualized support that empowers members to overcome challenges and achieve lasting stability.
At the heart of this effort are ODHS Family Coaches and CWP Specialist, who play a critical and personal role in the process. These dedicated professionals build meaningful relationships with members, taking the time to understand their unique circumstances, strengths, and aspirations. Through thoughtful conversations and ongoing support, Family Coaches and CWP Specialist collaborate directly to help uncover both goals and potential barriers that may be standing in the way.
Referral Links and Process
Use the CWP referral form to refer a new CWP member to our services.
Once you submit your referral form, CWP will assign a Resource Navigation Specialist to reach out to the new member and conduct an intake.
For new members being referred to Wellness services (DA, ME, RA, or MH) CWP will reach out to the new member to schedule an intake with our Medical Case Consultant or/and Behavioral Health Consultant.
